I enjoyed this book just as well as the first. A very unique story line. However, you never revealed why or how the donors and Milkers became institutionalized. How did they find themselves in such a predicament? Had they exhausted the abilities of their madams or were they just unlucky?

The Institution, donors and nurses were collectively a device for developing the character of Nurse Cummings.
Also handy for introducing new characters into the story line.
Otherwise, in so many stories over the years, the scene of Dominant woman milking subordinate males has been rather well worn by me.
I find that I can write about 10,000 words before encountering the need to significantly change the setting and/or bring in new characters. It's an amazingly consistent threshold for me, but since I am aware of it, I plan ahead so I do not write myself into a corner.
